Hey Everyone!!

 So, as a fairly new group, O.S.I.R.I.S. Paranormal is trying to get things figured out from an organizational standpoint, and we were hoping some of you guys could help! We are trying to figure out what kinds of forms, rules, guidelines, protocols, member medical forms, interview forms, questionairres, permission/request/release forms, investigation report forms, etc that we might need. Does anyone have anything that they could possibly send us?

We realize that a groups' forms and information are kind of like their "trade secrets", but we were looking for things to use as a template for our own forms, not looking for something just to copy and claim as our own. If anyone has anything they could send to us, whether their own forms or even ideas, it will be greatly appreciated. As always, good luck hunting, and we hope to work with you all in the future!
